"If he grabs you, don't pull away. It makes him hold tighter."
Claire Rowan looked up from the medication chart. The man giving the advice wore a shoulder holster under a navy jacket. He had the tired eyes of someone who had not slept in several nights.
"And if he stops breathing?" she asked.
"Call me."
"I'll call an ambulance. What's your name?"
"Marcus Bell. I run security."
"Then we both know our jobs."
Across the room, another nurse closed her bag. She had been scheduled to finish the shift with Claire, but her face had gone white after Dominic Varela threw a water glass against the wall.
"You can have my hours," she said. Her glance ran over Claire's fitted navy scrubs. "Maybe he'll be easier on you."
Claire understood the little smile that followed. She had spent years hearing that a woman with a soft waist and full hips looked more suited to a reception desk than a trauma bay. Tonight she had neither the energy nor the interest to answer.
"Leave the handoff notes," she said.
The nurse dropped them on a chair and hurried out.
Dominic lay in the middle of a bedroom larger than Claire's apartment. Three weeks earlier, somebody had shot him outside a restaurant. The bullet had crossed the muscle above his right shoulder. He should have been recovering. Instead, his legs had grown progressively weaker, and that afternoon his temperature had climbed past 103.
"Why is he still here?" Claire asked.
"He refused a hospital."
"When he could make that decision?"
Marcus did not answer.
Claire checked the chart again. The most recent entry described agitation and recommended sedation. It said nothing about the pain Dominic had mentioned in his lower back. His pulse was fast, his oxygen level poor, and his skin burned under her fingers.
She turned back to the first page. A handwritten note from the day before said he had refused to let anyone touch his legs. Another said he had kicked a tray across the room. Both entries treated the behavior as aggression. Neither recorded whether he could actually lift his feet afterward.
"Who wrote these?" she asked.
"The day staff."
"Did anyone check whether he could move his toes?"
Marcus frowned at the chart. "I watched him stand with assistance two days ago. Yesterday he couldn't get from the bed to the chair. I told the doctor."
"And?"
"He changed the pain medication."
Claire set down the chart. A change that quick had been reported, then buried under an easier explanation. She could see why a frightened nurse might leave this room. She could also see what leaving would cost.
"Who has medical authority if he's confused?"
"His brother Elias arranged the care."
"That wasn't my question."
Dominic's hand closed around her wrist before Marcus could reply.
Claire turned. His dark hair stuck to his forehead. In photographs he looked like a man who chose where everyone else stood. Fever had stripped that certainty from his face, leaving fear and anger in equal measure.

"Who sent you?" he asked.
"The nursing agency. I'm Claire."
"Elias sends everyone."
"Then Elias should have sent someone sooner. You're very sick."
He tightened his grip. Marcus took a step forward, but Claire held up her free hand.
"Dominic, look at me. Your heart is working too hard, and you need a hospital. I can examine you here while Marcus calls for help. Let go of my wrist."
"They keep giving me something."
"Who?"
His eyes shifted toward the medication tray. Then pain folded across his face.
"My back."
Claire felt his fingers loosen. She withdrew her hand and checked the tray. One vial had been opened without an entry in the chart. She set it apart and photographed the label with the agency phone.
"Has he complained about his back before?" she asked Marcus.
"Since the procedure."
"What procedure?"
"The surgeon cleaned the shoulder wound."
Claire looked at the discharge summary. The listed procedure concerned his shoulder alone.
"Who treated him?"
"Dr. Kessler. Elias uses him."
"Help me turn Dominic onto his side. I need to see where it hurts."
Marcus hesitated. "He hates being moved."
"Then we tell him first. Dominic?"
Dominic gave the smallest nod.
They moved him carefully. He swore once, sharply, and gripped the edge of the mattress. His shoulder incision looked clean. Claire examined the skin below it, then stopped at a narrow line near his waist.
At first she thought it was a crease. Under the examination light she saw the evenly spaced marks of sutures beneath a thin coating of adhesive.
"Bring me saline and the sterile swabs," she said.
Marcus fetched them. As Claire cleaned the line, a bead of cloudy fluid appeared between two stitches. The surrounding skin was hot and swollen.
"Is that from the bullet?" Marcus asked.
"No."
She took several photographs before covering the site with fresh gauze. The old surgeon in her wanted to name what lay beneath the incision. The nurse standing in this room knew that guessing would waste time Dominic might not have.
Claire checked the pulse at his ankle and compared it with the other side. She asked him to push against her palm. His right foot shifted weakly. The left did not move. He tried again, breathing through his teeth, until she told him to stop.
"That wasn't happening after the shooting," Marcus said.
"Write down when you first saw it," Claire said. "Use your own words. The hospital will need a timeline."
Marcus found a pen. For a man who had entered with a weapon beneath his jacket, he looked unexpectedly helpless holding it. Claire waited while he wrote. She had learned what happened when frightened people were told they could remember details later. Later, details became whatever the most powerful person in the room said they were.
"I need emergency transport and the name of the nearest hospital with spine surgery," she said. "Now."
Marcus reached for his phone.
"Don't tell Elias," Dominic whispered.
Claire bent so he could see her face. "I won't let anyone decide your care for you while you can speak for yourself. Did Kessler tell you he operated on your back?"
Dominic stared at her. For the first time that night, his answer was clear.
"Nobody touched my back."
He looked past her toward the open bedroom door. "Elias told me the weakness was in my head. He said if I kept fighting the nurses, nobody would come back."
Claire glanced at the empty chair where the departing nurse had left her notes. "Someone came back," she said, pulling the sheet gently over him.
In the hall, the private elevator chimed.
