Pierce said she had corrected Sofia’s medical record because Holt told her Sofia arrived alone.
Claire let the answer hang in the interview room. “Why would an officer who wasn’t there know?”
“You’ll have to ask him,” Pierce said.
“We have. He says he escorted her to the infirmary door.”
Pierce’s lawyer leaned forward. “Then my client was given inaccurate information.”
Elena watched from behind the observation glass. She had asked not to confront Pierce until Claire had obtained her account. The deputy warden’s initials appeared on two altered records, but the investigators still had to establish whether the changes had been mistakes, instructions from someone else, or a deliberate effort to hide what happened to Sofia.
Claire placed the original medical intake sheet beside a later photocopy. The nurse’s handwriting ended beneath the clinical observations. Another hand had added the line about Sofia arriving alone after the sheet was signed.
“Did you speak with the nurse before changing her account?” Claire asked.
“I clarified an administrative detail.”
“The nurse says no one asked her.”
Pierce looked down at the papers. “Then I should have.”
It was a small admission. Elena had learned to value those. Claire moved to the outgoing mail register and asked why Sofia’s envelope was marked returned without an address. Pierce said she had relied on Holt again. She could not remember seeing the envelope, and she could produce no instruction authorizing him to withhold it.
Price entered the observation room with a folder. “The maintenance records came in.”
The laundry corridor outage had been logged as an electrical fault at 9:17 p.m. A technician had tested the circuit the following morning and found no damaged wiring. The fault report had been closed without a repair. Attached to it was a request to disable the corridor camera for inspection earlier that same evening. Holt had submitted the request. Pierce had approved it.
“Could be routine,” Price said. “The camera may have needed work.”
“Was any inspection recorded?” Elena asked.
“No.”
Claire took the folder into the interview. Pierce said she had approved dozens of maintenance requests without personally checking each one. Claire asked who had scheduled this inspection. Pierce could not say.
By then the question of intent had become the last material uncertainty. The altered medical entry, the held letter, and the camera request each had an innocent explanation in isolation. Together they put Pierce repeatedly between Sofia’s complaints and any official account of them.
Elena requested an interview with Holt. Claire agreed on one condition: she would lead it, and Elena would ask only questions grounded in something already documented. Elena accepted.
Holt sat across the table with his union representative. He looked at Elena’s dry civilian clothes, then at the folder Claire opened.
“You came in pretending to be an inmate,” he said. “Everything you got out of that yard is contaminated.”

“The yard cameras recorded you,” Claire replied. “We’re here about the night Sofia Moretti went to medical.”
Holt folded his arms. “I walked her to the door. She was upset.”
“Why?” Elena asked.
“She accused us of stealing her mail.”
“Had you opened it?”
His representative touched his sleeve, but Holt answered. “I was told to hold it.”
“By whom?” Claire asked.
“I don’t remember.”
Elena slid Sofia’s copied letter across the table, open to the paragraph about trusting him. “She gave it to you because you promised the women named inside wouldn’t be exposed. Vera knew those names afterward. How?”
Holt’s eyes moved over Sofia’s handwriting. “Vera heard things.”
“Ruth says you handed Vera an envelope. She described a stain on the corner before she saw our photographs.”
“Ruth Calder did whatever Vera told her. You think she’s credible?”
“We checked what she could describe,” Claire said. “That’s why we’re asking you.”
Holt looked at the closed door. Then he said Pierce had wanted to know whether Sofia’s allegations implicated staff. He claimed he showed Vera the names only because Vera could identify which inmates were making false complaints.
“You gave the woman they feared the list of women who complained about her,” Elena said.
Holt did not answer.
Claire asked about the camera request. Holt said the image had flickered. The technician’s report showed that the camera was working when it was tested the next morning. That did not prove Holt caused the power failure. Claire said so aloud and asked him why there was no inspection report from the night he had requested one.
“I was busy escorting Sofia.”
“Who was with you?”
He stared at the folder. “Vera came into the corridor. She wasn’t supposed to.”
“Did you remove her?”
“I tried.”
“What happened to Sofia’s hands?” Elena asked.
Holt rubbed his palms together. “She held onto the door. I pulled her away. I shouldn’t have done that.”
Elena felt heat rise behind her eyes. She kept her voice level. “Who changed the record to say she arrived alone?”
“I told Pierce I’d taken her.”
Claire asked him to repeat it. He did. Holt could not explain why Pierce had written the opposite. His admission did not settle the cause of Sofia’s death, and Elena refused to let anyone speak as though it did. It established what Holt had done that night and what he had told his supervisor afterward.
At the end of the interview, Claire informed Holt that he remained off duty, that evidence of possible crimes would be referred for independent review, and that he must not contact the witnesses. He left without looking at Elena.
Pierce’s lawyer requested another interview after learning Holt had contradicted her. Pierce admitted she had added the medical line herself. She said she wanted the report to indicate that no officer had remained inside the treatment room. Instead, she had written that no staff member accompanied Sofia at all. She also acknowledged approving the camera request without confirming any work had taken place.
“Why did you mark her letter returned?” Claire asked.
Pierce looked at the envelope photograph. “Holt told me it couldn’t go out while staff allegations were under review.”
“Where is the review?”
Pierce could not produce one. She asked for a break.
That night the department removed Pierce from duty pending an independent investigation. Elena took no pleasure in the announcement. A suspension was an action, not an answer about everything Sofia had suffered.
Price brought her the draft summary of what they could prove. Vera had taken food and coerced women over months. Holt had passed along Sofia’s names, taken her letter, and physically moved her during the blackout. Pierce had altered records and failed to document the supposed review. Questions remained about Sofia’s medical care and whether the loss of food and the events of that night contributed to her death. Those questions were assigned to an independent medical review.
“I want the limits stated too,” Elena said.
“They are.”
She read every line. Then she signed her own account of the undercover operation, including her decision to sit on Sofia’s bench and the moment she chose to end her cover.
Before she left, Claire asked whether she would read a statement at the oversight hearing. Elena had spent months asking others to speak. When her turn came, she said yes.
